Second Dental Opinions is very easy to use. However, before you begin it is real important for you to read and totally understand our disclaimer. Since we do not have the opportunity to see you and perform a thorough clinical exam, the treatment plans we provide are based only upon the data you send us. In most cases, however, this should be enough to give you a very good idea where to go to obtain the kind of dental health you want and need.
Since we have no agenda we can be very straight forward in providing you with the information you need to make this all important health decision.
How Do I Get Started?
- Step 1: If you haven't already done so please read our disclaimer:
- Step 2: Print and Fill out our Medical/Dental History form as thoroughly as possible. Be assured that this information will be kept in the strictest of confidence based upon doctor/patient confidentiality.
In the Comments section at the end of page two please add any and all comments you feel may be important in providing you with a second opinion. Make this as complete as possible. For example, if you want to avoid removable dentures, we need to know it. If you are looking to keep the cost at a minimum, we need to know that as well. If you have a specific budget that would help but please understand that dental fees vary widely especially geographically and predicting costs can be very difficult.
- Step 3: Complete, sign and print our HIPAA form and deliver it to your current dentist so that you can have them send us (or provide you) with all of your most current patient clinical data (including progress notes, all charting both restorative and periodontal, study casts if available, clinical photographs etc.). The more data we have the better help we can be. If this data is available digitally it can be sent to us via email at patientrecords@seconddentalopinions.com and then go to our secure payment site to complete the transaction.
